DEADLINE TO REGISTER: 11:59 p.m. Arizona Time, Monday, February 12, 2024 Please Read All entrants must complete and submit this form. Required fields are noted with an asterisk (*). One registration form must be completed for each contestant – group entries will not be accepted. Please direct any questions concerning registration to hoopdance@heard.org. IMPORTANT Registration for children 13 years and under must be completed by a parent/guardian. Permission to perform will be confirmed via email/phone. Within three (3) business days, your Registration Form will be reviewed and verified. You will receive an email confirmation at the address you provide.
